For Licensed Professionals and Practices

Professional Practice Intake, Agreements & Compliance Packages

Much of what a licensing board later examines is documentation that was written long before any complaint existed. This work is for licensed professionals and professional practices — including, and especially, those facing no disciplinary proceeding at all.

What We Prepare

Documentation and Compliance Services

Prepared for a specific practice, in a specific profession, in a specific jurisdiction — rather than adapted from a generic template.

New Patient and Client Intake Packages

The complete set of documents a new patient or client receives and signs, assembled as one coherent package rather than a collection of unrelated forms.

Professional Service Agreements

Agreements setting out the services the practice provides and the terms on which it provides them.

Client Engagement Agreements

Engagement documents defining the scope of the relationship, what is included, and what is not.

Informed Consent Documents

Consent documentation appropriate to the services provided and the jurisdiction in which they are provided.

Treatment and Service Authorization Forms

Authorizations for specific treatments, procedures, or services.

Financial Responsibility and Payment Agreements

Documentation of who is responsible for payment, on what terms, and what happens when an account is not paid.

Cancellation and Refund Policies

Written policies covering cancellations, missed appointments, deposits, and refunds, drafted to be enforceable and clearly disclosed.

Privacy and Confidentiality Documentation

Privacy notices, confidentiality acknowledgments, and authorizations for the release of information.

Practice Disclosures and Acknowledgments

Disclosures a practice is required or advised to make, and the acknowledgments that record them.

Recordkeeping Policies and Administrative Procedures

Written policies covering how records are created, stored, retained, and released, and the administrative procedures that support them.

Compliance-Oriented Document Reviews

Review of documentation a practice is already using, against the obligations that apply to it.

Customized Practice Documentation Packages

A documentation set assembled for a specific practice, profession, and jurisdiction rather than adapted from a generic template.

By Practice Type

Documentation for Your Profession

Requirements differ by profession as much as by state. These are the areas most often addressed in each field.

Medical and Healthcare Practices

Patient intake packages, informed consent documentation, treatment authorizations, financial responsibility agreements, privacy and confidentiality documentation reflecting federal health information requirements, missed-appointment and cancellation policies, and clinical recordkeeping and retention policies.

Veterinary Practices

Client intake and new-patient packages, estimates and financial responsibility agreements, anesthesia and surgical consent, euthanasia authorization, boarding and hospitalization documentation, controlled substance recordkeeping policies, and deposit, cancellation, and refund policies.

Massage Therapy Practices

Client intake and health history forms, informed consent and draping policies, service documentation, sanitation procedures, independent contractor and booth rental agreements, and cancellation, no-show, and refund policies.

Mental and Behavioral Health Practices

Client intake and informed consent packages, treatment agreements, confidentiality and release-of-information documentation, telehealth consent and practice documentation, supervision agreements for associates and residents, termination policies, and clinical recordkeeping and retention policies.

Dental Practices

Patient intake packages, treatment plan and informed consent documentation, financial responsibility and payment plan agreements, missed-appointment policies, privacy documentation, and recordkeeping and retention policies.

Insurance Agencies and Agents

Client engagement and service agreements, disclosure and acknowledgment forms, privacy documentation, producer and agency administrative procedures, and file recordkeeping and retention policies.

Other Licensed Professional Practices

Engagement agreements, scope-of-service documentation, disclosure and acknowledgment forms, payment and cancellation terms, privacy documentation, and administrative and recordkeeping procedures appropriate to the profession and jurisdiction.

What This Is — and Is Not

Customized Documentation, Not a Compliance Guarantee

These are customized legal documentation and compliance-oriented services. Documents are drafted for a particular practice, in a particular profession, in a particular jurisdiction.

Engaging us to prepare or review practice documentation is not a guarantee of regulatory compliance. No set of documents can guarantee that a practice will be found compliant, and no document prevents a complaint from being filed.

Requirements differ by state and by profession, and federal law applies to some practices and not others. Documentation is prepared with the requirements applicable to your practice in mind, which is why it is not adapted from a generic template.

This work is available to practices that are not facing any disciplinary proceeding. Much of it is most useful before a problem arises.

Professional License Defense representation is available through defined stages of service. Contact our office to discuss your matter, the proposed scope of representation, and applicable fees.

Preparing or reviewing practice documentation does not create a guarantee of regulatory compliance, and no document prevents a complaint from being filed or determines how an authority will view a matter. Applicable requirements depend on the profession, the jurisdiction, and in some cases federal law. Engaging us for documentation work does not constitute representation in any disciplinary proceeding unless separately agreed.
